Your participants are encouraged to begin using the portal by taking our custom Health and Productivity Assessment (HPA), which identifies their specific risk factors. The data from the assessment is used to develop an Action Plan with recommended programs based on the participant’s goals, risks and preferences. Individuals at higher risk may also be identified for more robust interventions such as telephonic health coaching and disease management.
