The Alere Diabetes Program helps people with diabetes to control the disease and prevent costly medical events in the future. We help participants understand their illness and the serious health effects from not adhering to care plans and alter unhealthy lifestyle behaviors that contribute to their condition.
Alere provides configurable solutions designed to address the high-cost, high-risk population, as well as solutions that address all segments of a customer's population.
Our clinicians proactively contact participants to make sure they are adhering to their treatment plans that may include medications, a healthy diet, exercise to control weight and both at-home and outpatient testing for blood glucose, cholesterol and fatty acids.
Our program has repeatedly demonstrated that teaching individuals to be proactive in managing their disease leads to reduced hospitalizations and emergency room visits, increased productivity and less absenteeism.
